Children and Youth Intensive Case Management

The goal of Children and Youth Intensive Case Management Program is to provide intensive case management services to seriously emotionally disturbed children and families. The Intensive Case Manager meets with each family to assess the needs of the identified child and the family in an effort to coordinate services and advocate with service providers. The goal of the program is to reduce the risk of in-patient hospitalizations while insuring needs are being met in the least restrictive environment. Families also receive training to assist in handling the difficult behaviors exhibited by many emotionally disturbed children.

The Harbour Program Intensive Case Management staff work very diligently to ensure that the parents of children receiving Intensive Case Management services are active participants in planning the services for their children.
